Yes, that looks simple.
With CAPI and Eicon cards it would be like

[faxin]
exten => fax,1,capicommand(receivefax|<filename>)
exten => fax,2,System(faxreceived.sh ${FAXID} ${FAXPAGES} ${FAXRESOLUTION} ${FAXRATE})
exten => fax,3,Hangup
