I've used the NetIQ stuff (on a weekly rental basis) and its pretty good
when used as a comparison tool. By that I mean making a simulation run,
adjusting parameters, and making another simulation run.

FWIW, all of the major US carriers have some sort of VoIP backbone in 
production use, but its isolated from the Internet. Sprint, as one 
example, has been using it in their long distance backbone facilities 
for years, which is now migrating to handle the NexTel traffic.
