This is incorrect.  It should be:

[creat-in]
exten => 400,1,Dial(IAX2/400,20,tr)
exten => 400,2,Voicemail(u400)
exten => 400,3,Hangup


Better yet:

exten => s,1,Dial(IAX2/400,20,t)
exten => s,n,NoOP(Dial Status: ${DIALSTATUS})
exten => s,n,NoOP(Hangup Cause: ${HANGUPCAUSE})
exten => s,n,Goto(s-${DIALSTATUS},1)

exten => s-CHANUNAVAIL,1,Voicemail(u400 at default)
exten => s-NOANSWER,1,Voicemail(u400 at default)
exten => s-BUSY,1,Voicemail(b400 at default)
exten => s-CANCEL,1,Congestion()
exten => h,1,NoOP(Hungup)
