Hi Chuck!
In sip.conf you have defined context=default in general or some of user/peer/friend section. So, Asterisk tries to send call to default context which you don't have in extensions.conf.
You can
1. in sip.conf change from context=default to some other context that exist in extensions.conf or
2. create default context in extensions.conf
