I have since added fxs_ks=1 and channel => 1

This has not fixed the problem.  I do notice a warning on the reload of
asterisk.

Those need to be in the section [channels] and be followed by a

  channel => 1

to actually have any effect. You also must set signaling (signalling =
fxs_ks; in your case).
