With a little modification the sample that I provided would do exactly that:

[internal-phones-only]
exten => _*21.,1,Answer
exten => _*21.,2,DBPut(DNDFWD=${CALLERIDNUM}/${EXTEN:3})
exten => _*21.,3,Playback(vm-goodbye)
exten => _*21.,4,Hangup
