I have a similar or maybe the same issue.  I have found no solution for it
but to just ignore the new hardware prompt.  I saw someone say in an earlier
email that the pci subvendor id seems to change at random on reboot, causing
the card to be redetected.  I also find that if the card is connected to a
TE410p or may be just if it is connected to a PRI in general (E1) that the
card drivers won't unload and then will fail to redetect the card on reboot.
I have remote access cards in all my servers with these and power cycle the
server when the drivers need to be reloaded.  Not an ideal situation but
acceptable for my purposes.
