I got similar issues, Im running P0S3-07-2-00 load
In your tftpboot folder in your TFTP server make sure you have these files:

CTLSEP000D651CF3FB.tlv
SEP000D651CF3FB.cnf.xml

SIP000D651CF3FB.cnf

[root at server tftpboot]# cat CTLSEP000D651CF3FB.tlv
P0S3-07-2-00
[root at server tftpboot]# cat SEP000D651CF3FB.cnf.xml
<Default>
   <callManagerGroup>
       <members>
           <member  priority="0">
               <callManager>
                   <ports>
                       <ethernetPhonePort>2000</ethernetPhonePort>
                   </ports>
