Thank you for that, it was very much appreciated.

Also instead of putting a whole bunch of hints in, how might I go about putting a cluster of SIP extensions in the hint off the PSTN situation?

Could you also maybe throw me a couple of hints what the 

exten => 691,1,Macro(stdexten,SIP/bt-karen)

Macro portion I have seen in some examples but I am not sure what it does.
