The Annex B option seems to be common in Europe. The Swedish incumbent, 
Telia, also uses that option. Actually, their implementation document is a 
lot easier to read than Deutsche Telekom.

Clause 5.2.20 describes CLIP handling, including the behaviour you 
observed. The receiving subscriber has the option to only received the 
unscreened number or to receive both the screened and unscreened numbers.

It seems resonable that Asterisk should be extended to handle the case 
where both occur and pass both to the dialplan.
