[Asterisk-Users] Unexpected On Hook event

I have an asterisk configuration in which the external analog line dials in 
through an FXO.
The dial plan has Zap1-1 dial Zap2-1 after playing the congratulations 
message. A few
seconds after Zap2 answers and begins to send audio messages across the 
line, an
On Hook(1) event for Zap2-1 seems to occur out of the blue, and the 
connection goes away.
How could this happen?
