I'd vote for a card.  You'll have the best results using a card with the 
HFC-S chip and Kapejod's driver (www.junghanns.net).  Next best is an AVM 
Fritz! card with the CAPI interface and Kapejod's CAPI driver.  

The BT Speedway ISDN PCI card is an AVM Fritz! card - cheap on EBay.
