Interesting, would this be considered a bug or is it rather intentional?

IAX does not correctly set the HANGUPCAUSE for a LOT of things.  Look at

DIALSTATUS or look at the dial-result macro on 
http://www.fnords.org/~eric/asterisk/downloads/macros.inc
