[Asterisk-Users] Non-working module on TDM400P?

Hi all,

I was wondering if anyone had any pointers on how to determine whether or
not a module has gone wonky on the TDM400P?

I have a 2 FXO (channels 3 and 4) and 2 FXS unit (channels 1 and 2). The
bad (?) module in question is the FXO module on channel 3. I can't dial
in to or out of that channel; dialing in gives a busy signal, dialing
out just shows * hanging around after attempting a native bridge.

Some notes on my setup:

1. zaptel.conf and zapata.conf should be properly configured, since *
works well receiving calls on channel 4.

2. LED on all channels show green

3. unloading the zaptel and wcfxs modules, then reloading them again
doesn't fix anything. in fact, after a fresh reboot of the machine, i
can only get the busy signal and nothing else on channel 3.

4. I've made sure the board is running on an IRQ that isn't used by
anything else. lspci -v, /proc/pci, /proc/interrupts and /proc/ioports
all show data which corroborates that things are okay.

5. dmesg shows:

zaptel: no version for "struct_module" found: kernel tainted.
Zapata Telephony Interface Registered on major 196
Freshmaker version: 63
Freshmaker passed register test
Module 0: Installed -- AUTO FXS/DPO
Module 1: Installed -- AUTO FXS/DPO
Module 2: Installed -- AUTO FXO (FCC mode)
Module 3: Installed -- AUTO FXO (FCC mode)
Found a Wildcard TDM: Wildcard TDM400P REV E/F (4 modules)
Registered tone zone 0 (United States / North America)

6. lspci -v shows:

and nothing else is running on IRQ 3.

7. some errors show up on /var/log/messages intermittently:

Oct  5 16:19:15 demo kernel: Power alarm on module 2, resetting!

8. ztmonitor on channel 3 shows absolutely zero activity when attempting
to receive an incoming call or when attempting to make an outbound call
from asterisk

so really the only thing that's telling me the module is not working
properly is the fact that i can only get a busy signal when i dial in to
channel 3 on the card.

anyone has any suggestions to confirm the module's dead? so i can
proceed with returning the card.
