[Asterisk-Users] TDM11B auto configuration issue

I'm having problems getting a TDM11B (green FXS module in slot 1 and the
red FXO card in slot 4) to configure at boot time. zaptool and ztcfg all
indicate that the card is working fine and with no errors, but the log
file has the following messages:

The autoconfig of the FXS module isn't working and a spurious USB FXS
interface driver gets registered for some reason (though zaptool doesn't
show it).
While I can get a phone set to ring on the FXO port and the simple
switch starts up when it goes off-hook, I don't get dial-tone and cannot
dial out.
I've checked out the power cable it's using and it seems to be fine
(11.97 v and 5.1v)
Anyone have any ideas on why it wouldn't auto configure, or how I can
manually configure it?
