http://voip-info.org/wiki-Asterisk+cmd+LookupCIDname
This uses Asterisk's internal database.

If you want to store in an external database, then this will do the trick:
http://asterisk.bkw.org/other/app_dbodbc.c

Used in a similar way to:
http://voip-info.org/wiki-Asterisk+cmd+DBput
