Perhaps the DTMF is being passed "inband" instead of "out of band" and
the call is using a compressed codec, such as GSM.

See "dtmfmode" in sip.conf and iax.conf.  Valid values are "inband",
"info" and "rfc2833".  I suggest using rfc2833 (out of band).
