[Asterisk-Users] Random Hangups

Is anyone experiencing random hang-ups while in a call? I'm using Cisco 
7940s with Catalyst 2950s, QOS enabled. The PRI channels (Zap/g1) are 
connected to a Digium T100P.
In the call, asterisk is issuing a hang-up, not the other end.

example 1:

     -- Executing Dial("SIP/Derek-1-5a13", "Zap/g1/18008193902") in new 
stack
     -- Called g1/18008193902
     -- Zap/1-1 is ringing
     -- Zap/1-1 answered SIP/Derek-1-5a13
     -- Hungup 'Zap/1-1'
   == Spawn extension (default, 918008193902, 1) exited non-zero on 
'SIP/Derek-1-5a13'
     -- Executing Hangup("SIP/Derek-1-5a13", "") in new stack
   == Spawn extension (default, h, 1) exited non-zero on 
'SIP/Derek-1-5a13'

example 2:

     -- Executing Dial("SIP/Derek-1-44a6", "Zap/g1/18776334236") in new 
stack
     -- Called g1/18776334236
     -- Zap/1-1 answered SIP/Derek-1-44a6
     -- Hungup 'Zap/1-1'
   == Spawn extension (default, 918776334236, 1) exited non-zero on 
'SIP/Derek-1-44a6'
     -- Executing Hangup("SIP/Derek-1-44a6", "") in new stack
   == Spawn extension (default, h, 1) exited non-zero on

Normally when the other person hangs up I get this:

     -- Channel 1, span 1 got hangup
     -- Hungup 'Zap/1-1'

Any ideas?
