Have you defined /etc/asterisk/capi.conf?

Mine looks like this, phone numbers changed, however:

There is a sample capi.conf file supplied with the capi drivers. You can 
also use a * on incomingmsn to accept all MSN numbers. I have some set 
as fax lines however so I don't mentioned them here as I want Asterisk 
to ignore them and Capi4Hylafax to accept them.
