I wasn't required to enter a proxy, but I could.  Try
using the default sip.conf and use the config info
above.  If you get that figured out, and DTMF works
for you -- let me know.

All the config I can remember on sjphone is friendly
name is your username, like sip host is your asterisk
ip, plus your auth info.  Not at home now so I can't
check.  Let me know how it goes.
