Sorry....   I wasn't using that section and conveniently forgot about
it.  Commented out in the attached patch.

inband dtmf was never actually hooked up to any code before.  You should
only use inband dtmf if the device does not send the mgcp NTFY messages.

Hmmm....   If (dtmfinband) { Should I ignore NTFY messages for digits? }
