I never even noticed it worked fine without speakerphone.  Thanks for
pointing that out :)

Upgrading the firmware and changing the DMTF to Info corrected
everything.  Thanks so much!

using speakerphone with dtmf=inband? 
that's caused by a too loud dtmf tones when on
speakerphone.

upgrade to latest firmware (.72) and use
dtmf=info or rfc2833, should to the trick.
