Don,
Thanks very much for the suggestion. That fixed the problem.
It's very strange because I should be providing clocking on the PBX PRI
port but as soon as I changed my span configuration to 1,0,0,esf,b8zs it
came up and worked perfectly.

This may have nothing to do with it but have you verified your timing?
Make sure one end of the T1 is using an internal clock and the other
end is using timing off of the T1.
