I use iconnecthere, and lately had been struggling ever to get a call to 
go through without the dread "480" message.

I just grabbed the CVS and built it, and so far have made two 
iconnecthere calls.  They both went through, but I'm wondering about two 
things:

1. I don't know what the fancy name is, but I've never gotten "ring in 
your ear" feedback when an icconnecthere call is being set up.  Should I be?

2. So I faked it with the "|r" suffix on the extension, but now ("now" 
meaning with the last few days' CVS) once the callee picks up and the 
CLI shows the channels bridging, there is a noticeable (1.5-2sec) delay 
before the parties can hear one another.

Taking the "|r" away fixed that delay, but I don't get any ring 
indication, which is eerie.

I'm behind a NAT on this, if that's germane.

I'll keep making some calls, but in basic testing, it does appear as if 
now I can call that way reliably.
