[asterisk-r2] DTMF issue after migrating Unicall -> OpenR2

There are some reasons for ZT_DIAL to fail in the zaptel drivers.

1. kmalloc() fails.
2. copy_from_user() fails
3. There is no tone zone loaded for that channel.
4. Transmit buffer is full.
5. Invalid operation specified.

All of them are unlikely or just not possible given the information
you provided, except for no tone zone. If you check /var/log/messages
I bet you will see a warning saying zaptel: Cannot dial until a tone
zone is loaded.

Is that the case?
