[asterisk-gui] Enhancements to CDR Viewer

Brandon, I fixed a bug in parsing the CSV in Master.csv (that only appears
on Internet Explorer) and have added some more refinements to the CDR
Viewer.  Please take a look at the "-v3" patch in this feature request...
http://bugs.digium.com/view.php?id=13825   (use a -p1 flag on the patch
command when applying in the asterisk-gui source directory). Patch was built
from diff to svn version 4102.
Still very interested in your thoughts on these enhancements.
