Any heads up on what this is or how to fix it

[Mar 31 20:31:10] WARNING[2947] app_voicemail.c: Unable to read password
I don't know what i've done, i think it worked before
I can ring phones but no audio in either direction, voicemail could log
in cos it could read the password
I rolled back to a backup and all is working fine but what did i do?
