This issue has already been fixed in the latest svn branch 1.4. You can fix this manually by changing the reference(s) of "IAX/" to "IAX2/" in the config. Alternatively, you can "upgrade" asterisk to the latest svn branch 1.4.

Subject: [asterisk-gui] new iax extension
Hello,
I add a new iax extension to my asterisk 1.4 server using the asterisk-gui web interface. When I attempt dial this new iax extension, I am getting the following error message:
-- Executing [2021 at numberplan-custom-1:1] Macro("SIP/2001-09382cb0", "stdexten|2021|IAX/2021") in new stack

How can I make the iax extensions iax2 extensions or fix this issue?
Thanks
Eric
