Yes it looks like asterisk does not take in account when the hold key of cisco is pressed... but now I setup rf2543hold to true, and it seems to wrokin well.

So, my question is to know if there's a best way to address music on hold in asterisk?

Maybe the new way is bug in *?

I have a similar problem that may be related to yours:

I am using Asterisk 11.2 with Snom phones. My customer did not complain about the "Hold" 
functionality, maybe because they don't use it. I suspect the directmedia setting has some influence, but haven't tested this yet.
