At AstriCon I learn't that one integrator has a customer with ~20,000 grandstreams deployed, and the description was that 'the BLF's stop working for ever'.

In the current code base, there is code already there to prevent an MWI expired subscription notification being sent, probably for exactly the same reason as I'm proposing for the BLF.
I'm just extending that test to not send final notification when p->subscribed == DIALOG_INFO_XML
