I have further revised the patch for type=service

The patch has support for the following features:
- matching on callbackexten/registered-contact
- default registration for type=service
- required registration matching
- extracting DID exten from to-header rather than request uri
- nat-friendly registrations in peer definition

Some successful testing of type=service has been performed but I would
welcome other feedback to make it suitable for 1.8 trunk.
