Looks to be a problem with Ubuntu:

Inconsistency detected by ld.so: dl-close.c: 731: _dl_close: Assertion `map-&gt;l_init_called' failed!

I've been seeing this issue under a local VM instance, but this is the first time I've seen it on a remote agent.  Any suggestions where to start?  It's random from what I've been experiencing.
