I have fixed a lot of issues in chan_iax2 since 1.4.11.  Please give the latest
code in the 1.4 branch a try.  Also, please try it without disabling any of the
code.  It should be working fine.  If not, I want to know about it.

There will most likely be a release this week with all of these fixes in it.
