Actually it is configurable.
The patch addes the option rtpip to sip.conf.
I just need to add a rtpip=auto to it, to disable the patch.
When used, you just set rtpip to the external ip you want to use.

And the patch is just a hack, not something we would implement. It  
needs to be configurable.

I would recommend looking at the asterisk-security distribution for  
inspiration, where we actually handle
remote RTP by using the midcom protocol.
