There are known issues with IAX2 video negotiation. I've sent a
message to these lists a while ago about this, you can check it out

Right now I have a quick and dirty patch to Asterisk 1.4.0-beta3 that
solves the negotiation issue at least for some cases (quick and dirty
being key words here). We should come to a resolution as to what is
the correct way of negotiating video in IAX2, so all of us can go and
actually implement it. I'm not sure who are the persons in charge of
chan_iax.c but if you're reading this, please provide some feedback.

I've merged Neil's video switch code into the latest experimental
app_conference. The future versions will be based on it
