[Asterisk-Dev] Minor typo and a few files left open

Just noticed a typo in the help for app_dial (defiend instead of
defined)

 

And also using lsof to try and find where safe_asterisk is still running
on my system, noticed that app_rxfax is leaving the debug files open
(fax-rx-audio..... and fax-tx-audio....) and also asterisk is leaving a
deleted iaxy-bin file open in /tmp.
