[Asterisk-Dev] TDM analog cards stop being able to detect off-hook after a few weeks

We presently have 3 TDM cards installed in separate systems. Each one
works beautifully (great voice quality) for about 2 weeks then stops
being able to detect off-hook. By this I mean that the FXO modules stop
being able to detect incoming phone calls and the FXS modules ring the
phone but the phone carries on ringing after someone picks up the
handset. Reloading the wcfxs seems to sort the cards out. Please note
that asterisk carries on working for calls not involving the TDM cards.
A "zap show channel" of a card that has failed is shown below.

I can't see anything unusual about it. Anyone got any ideas?
