You can keep the phone at a distance of 10m maximum from the BT dongle, so
this must not be an issue even in the server rooms.

You do not need to use IAX/SIP or some other protocol for the
Asterisk/BT/GSM interface.
You can do it directly:
Asterisk server --> BT dongle --> GSM phone through a dedicated channel
like chan_bt
You can integrate SMS functionality in the chan_bt too. This is not
difficult.

You can consider BT channel as an OSS channel with the control data
passed through the serial interface.
