rat does this for multiparty rtp streams.  You assign a spatial 
"location" to each participant, and then it uses ordinary stereo output 
for location..  I've never tried it, though..

I bet I could hack something like this into iaxclient and 
app_conference..  just send a test frame or other signal whenever the 
active speaker changes..
