Interesting issue.  In this case, there's only 1 channel and no callerid/connectedline at all.  It gopt me thinking though... I think we're going to have a similar issue when there are 2 channels but callerid_privacy is set to "prohib".  In this case, the From gets set to Anonymous and PAI and RPID are suppressed on the outgoing INVITE BUT the connected line is still set on the channel.  We _could_ therefore still send an attestation but should we?
