Just browsed past this while looking for something else, and I don't know at all whether this is related but it seemed like maybe it might be - a hangup somehow ending the DTMF and preventing Dial from seeing that?: https://github.com/asterisk/asterisk/blob/b4347c486150653ec7ce1d129e8f9017c69344da/main/channel.c#L3291
