Patch Set 2:

I'm not cherry picking this change yet.  There are bound to be lots of comments. :)

I'm also trying to figure out a good way to test.  I'd have to run a whole bunch of testsuite tests in the same asterisk instance, change the config files, reload res_pjsip, then run the same tests again.  I could probably do it for 1 scenario, say outbound registration, but I don't think it would tell us much.  I'm open to ideas.
