Patch Set 2:

Shutdown isn't a problem, you're right. Deadlocked threads during module unload/reload are more of a problem (though luckily modules are not typically unloaded during normal operation) and people should report them as bugs instead of Asterisk just silently leaking the threads. I'll put in a warning when a thread is leaked so that deadlocks are more likely to get noticed and fixed.

I'll update the patch tomorrow with return -1 as you suggested and check if any changes to cleanup are necessary.
