Patch Set 5: Code-Review-1

1) There are numerous coding guideline issues with the chan_iax2.c changes in the patch.

2) Several of the added debug messages in chan_iax2.c are not useful to be left in.  They were only useful for debugging this problem.

3) The changes in sched.c need to be removed as they only add an unconditional debugging WARNING message that has the potential to spam the log.
