You haven't given enough information about the configuration. If chan_sip caching is enabled then yes, you have to reload. There's also other cases where you will have to reload (such as starting qualify). This is not going to change as chan_sip receives no such changes and will be removed.

The chan_pjsip module will also behave the same way for some things as some things require state to exist, and in order for it to know about changes you have to tell it.
