I've put the attached patch up for review as it seems like it might have fixed this problem (even if later found to not be the exact cause it's something that still needed to be fixed).

Note, once the patch is submitted this issue will auto-close. If later it's found the issue is still a problem then commenting as such will reopen the issue.
