We have not had a chance to revisit this yet, but we are 100% sure at this point it is related to pjsip peers in realtime. With identical configuration, just moved to the filesystem to create/update peers, this bug doesn't occur. If anyone else has cycles to look at the issue, we could certainly test patches, or we'll get back to this at some point soon here, I hope.
