How often does the problem occur? For instance, 1 out of 3 times, half the time?

Could you please describe in more detail the basic scenario, and expectation of a successful vs a failed attempt.For instance, based on the call file I'm assuming it's expected attempt to call the specified extension 2 times and then fail, but it appears to not fail?

Also could you enable at least debugging level 1 (how to here [1]), and attach a "good", and if possible also a "bad" execution?
