This has been an interesting one, and I'm not entirely sure if the patch I've created will fix your problem. After a deep dive into the code it is hard to find a place where your situation could occur. Anyway I have created a patch that _might_ fix things.

If you can please [download the patch|https://gerrit.asterisk.org/changes/asterisk~11697/revisions/2/patch?zip] [1], and apply it and test it out.
