OK I reproduced this on *13.21
Sorry for the delay but I lost access to the environment where I could reliably reproduce this problem and just got it back.
I have to forward the incoming call and I have to enable mixmonitor recording flag in order to reproduce this on 100% of calls.
Compiled with DONT_OPTIMIZE BETTER_BACKTRACES DEBUG_THREADS
Output of ast_coredumper and core show locks and Asterisk CLI is included in attached deadlock_2018_05_09.rar file
